determined by the President or presiding officer. To speak during this item you must

August 15, 2012 at 6 P.M.
Svore Municipal Building Boardroom
The Board of Directors of the Trophy Club Municipal Utility District No. 1, of Denton and Tarrant Counties,
met in regular session Wednesday, August 15, 2012 at 6 P.M., in the Boardroom of the Administration
Building, 100 Municipal Drive, Trophy Club, Texas 76262. The meeting was held within the boundaries of
the District, and was open to the public.

President Sanders announced the date of August 15, 2012, calling the meeting to order and announced a
quorum present at 6:00 p.m.
WORKSHOP SESSION
1. Discussion of the 2012-2013 Interlocal Agreement for Employee Services Contract (SEMO)
with The Town of Trophy Club
District Manager McKnight gave the Directors the revised SEMO agreement which listed IT at $78,461,
Human Resource/Payroll Service $32,196 which (broken out between the Fire Department and MUD),
Grounds Maintenance $9,025, and a charge to the Town for their portion of Janitorial Services of $10,071.
Additionally, $10,000 was added into the Fire Department budget for Public Information Officer. This brings
the MUD SEMO cost to $93,513 and Fire SEMO cost to $26,098, for a total of $119,611.
Vice President Thomas inquired as to what would be included in the Human Services/Payroll portion of the
agreement. District Manager McKnight stated that this is for the processing of bi-weekly payroll and other
benefits enrollment, and and cost for software.
President Sanders believes that we are late in the cycle and there are many unknowns if the MUD were to
take over the payroll and entirely cancel the employee services agreement with the Town.
Secretary/Treasurer Carr is concerned that the Town did not seek bids for healthcare coverage.

Director Moss stated that he would not be able to cast a vote for the SEMO agreement or the budget until he
receives a report from the SEMO committee with recommendations to the Board.
Secretary/Treasurer Carr stated that he would like to bring all services in house and submit the ninety-day
notice to revisit and/or cancel the contract.
2. Discuss and take appropriate action regarding Directors' responsibilities and duties to other
Directors and the District Manager regarding direction, actions, negotiations, information, and
positions of the Board. (Carr)
Secretary/Treasurer Carr stated that he would like to make sure all Directors agree on the duties and
responsibilities of each Director. Director Carr refers the Board to the Texas Water Code 49.054 which
states that the President presides over meetings, the Vice President presides in the President’s absence,
and the Secretary maintains the records.
Director Carr further referred to the MUD ethics order, which also states the roles and responsibilities of the
Directors. Carr stated that the day-to-day operations of the MUD are to be run by the District Manager.
Further, Directors are not to individually direct staff or District Manager McKnight. If a Director does not
approve of how the District Manager handles day-to-day business, the board must come together to give
her direction.
Director Moss stated his understanding of the roles of the President is to oversee the meetings and sign
paperwork primarily. Director Moss stated that no Director is to give direction or determine procedure
without the Board’s approval. There have been instances where this is not being followed.
Vice President Thomas reminded the Board that they have agreed in the past to be respectful of the District
Manager’s time. They also agreed that no Director was to be roaming around in the office without the
District Manager’s knowledge.
Secretary/Treasurer Carr asked the Board to review the ethics policy and remember why they are here and
how they are to act as individuals and as a board.
3. Consider and take appropriate action to review proposal for employee benefits (McKnight)
District Manager McKnight reviewed the insurance benefits comparison with the Board.
Vice President Thomas stated that he is concerned that the Town Manager did not go out for bids for
employee benefits when the comparisons received by the District Manager clearly show a cost savings and
are significantly better benefits.
President Sanders stated that the original SEMO agreement does state that the District Manager should
have been involved in the benefits decisions and this is one of the reasons that the MUD is looking to
change the SEMO.

REPORTS & UPDATES
5. Receive District Manager's Report (McKnight)
a. Update - Water System Operation

District Manager McKnight stated the contractor at the roundabout hit a sewer line during construction and
the method they used to fix it was unsuccessful and left some bellies in the pipe. The contractor will attempt
to repair the 8” line again tomorrow and they are aware that if this is not successful, they will have to open
cut to repair the section of line that was damaged. The contractor will also be completing the tie in to the
new water line.
Secretary/Treasurer Carr is concerned that the MUD has paid $187,500 for a Town project in which we
have had a supervisor on site for the past month. He would like staff to track personnel time spent at the
roundabout sites.
6. Receive Finance Update (Gonzales)

Senior Accountant Gonzales stated that for the month of July there were 21 new PID connections, a total of
242 this year, for a total of 867 since May of 2008. Per the GASB 54 policy adopted by the board in FY
2011, the Unallocated Fund Balance to Expenses percentage should range between 25% and 35%. Mrs.
Gonzales stated that the percentages as of 7/31/12 were 49.9645% of the Adopted Expenses and
43.9176% of the Amended Expenses.
Utility Billing continues to see an increased number of customers using credit card draft and online payment
options. The number of customers on the monthly disconnect list continues to decline.
Senior Accountant Gonzales updated the Board regarding the deposit refund process. The current total
write off of $8,569.94 was the amount left after the customer deposits were applied. Of the $8569.94, MUD
Services accounted for $6,253 and Town related services accounted for $2,317 of the total. Senior
Accountant Gonzales recommends that we do not refund customer deposits.
Director Carr will go along with District Manager McKnight and Senior Accountant Gonzales
recommendation.
District Manager McKnight stated that the bad debt would go up if the deposits were to be refunded.

Senior Accountant Gonzales provided the Board with a delinquent taxes report from Perdue, Brandon,
Fielder, Collins and Mott, LLC.
7. Receive Update on water leak report status (Womack)
Utility Superintendent Adrian Womack stated that the Water Department started running a report that
indicates customers that could have a leak. The reports showed in May – 345, June – 337 and in July –
363 houses that could have a potential leak. The report picks up those meters that have not stopped
running for one hour in a 24-hour period. Of the three months of reports, 212 homes showed up on each
month’s report consecutively.
Superintendent Womack stated that this month they completed data logs on 15 of the reported homes to
see if the leak was a true leak and they found there were leaks at those homes.
District Manager McKnight recommends that a postcard be sent to residents informing them of a potential
leak and the most common causes of leaks. She stated this will cost about $1,800 per year for postage plus
postcards and staff time. She stated she will get a procedure in place as soon as possible.
8. Receive Fire Update (Thomas)

Fire Chief Thomas stated the department took 101 calls for the month of July and 18 of those calls were for
mutual aid.
Secretary/Treasurer Carr would like to revisit the idea of implementing volunteers to the Fire Department.
Chief Thomas stated that the Quint truck final bill was $16,000, of that amount, $2,700 was preventative
maintenance. TML is going to review the final bill for the Quint repairs.
Secretary/Treasurer Carr inquired as to the status of the lighting at the station. Chief Thomas stated that a
tentative quote for lighting was obtained for $3,400. Chief believes that when the ladder truck is returned,
that department personnel would be able to install the lighting.
9. Receive Town Council Update (Rose)
Councilmember Rose is absent; President Sanders read Councilmember Rose’s update to the Board.
10. Receive Conflict of Interest Questionnaire and Affidavit Disclosing Substantial Interest in a
Business Entity or in Real Property from Director Sanders (Sanders)
President Sanders submitted to the Board a completed Conflict of Interest Questionnaire and Affidavit
Disclosing Substantial Interest in a Business Entity as requested by the Board when considering the ethics
complaint filed against him.
11. Review proposed FY 2013 Budget (McKnight)
Changes to FY 2013 Proposed Budget
Add reserve funds to I&S of $345,000.
Schedule GF-H (Salary Worksheet): Fire Department personnel lowered to 1.5% COLA and a merit
raise not tied to a percent.
Schedule GF-L (SEMO): Amend budget after SEMO committee meets.

12. Consider and take appropriate action to approve the July 2012 Financials (Gonzales)
Motion made by Secretary/Treasurer Carr, seconded by Vice President Thomas, to approve the July 2012
financials.
Motion carried unanimously
The Board recessed at 8:50 p.m.
The Board reconvened the meeting at 8:59 p.m.
President Sanders asked the board to move item #16 ahead of item #13. The Board agreed and the item
is moved.
16. Consider and take appropriate action regarding TCEQ enforcement letter dated July 23, 2012
and options related to same (McKnight)
District Manager McKnight and Wastewater Superintendent Karl Schlielig, presented a power point
presentation explaining the process of wastewater treatment, corrective actions being taken to comply with
the discharge permit, and options available regarding penalties for past discharge limit violations.
Motion made by Vice President Thomas, seconded by Director Moss, to approve Agreed Order and to
submit an application for a Compliance and Custom SEP to TCEQ.
Motion carried unanimously
The Board thanked District Manager McKnight, Wastewater Superintendent, Karl Schlielig and staff, for
their hard work and being pro-active.
13. Consider and take appropriate action to adopt Order No. 2012-0815 District Purchasing
Policy (McKnight)
The Directors would like to have District Manager McKnight review the purchasing card limits for staff and
lower those limits if she feels they should be.
Motion is made by Secretary/Treasurer Carr, seconded by Vice President Thomas, to adopt Order No.
2012-0815, District Purchasing Policy.
Motion is amended to change item #11 in the order to insert “the District’s Finance Manager and/or Senior
Accountant shall not”.
Motion as modified carried unanimously.
14. Consider and take appropriate action to adopt Order No. 2012-0815A Adopting Amended and
Restated Investment Policy (McKnight)
District Manager McKnight stated the change in the policy is to allow a letter of credit in lieu of pledged
collateral. This change was made in the 2011 Legislature and will save staff time on a monthly basis.
Motion made by Secretary/Treasurer Carr, seconded by Director Armstrong, to adopt order No.
2012-0815A Adopting Amended and Restated Investment Policy.

15. Consider and take appropriate action to award the FY 2013 Contract for Bank Depository
(McKnight)
District Manager recommends that the MUD stay with First Financial for Banking Services. The new
Contract for Depository services would be in effect from October 1, 2012 through September 30, 2015 with
the option to extend the contract for an additional two years.
Motion made by Secretary/Treasurer Carr, seconded by Vice President Thomas, to approve the proposal to
accept and award the FY 2013 Contract for Bank Depository to First Financial Bank.
Motion carried unanimously
17. Consider and take appropriate action for September evaluation of District Manager to
include setting goals. (Sanders)
President Sanders would like to know how the Board would like to evaluate District Manager McKnight.
Director Armstrong would like to have District Manager McKnight set goals for herself. Director Carr would
like to have the Board determine an amount for the merit increase for the District Manager so it can be put
into the budget. Director Carr would like to set an increase of $5,000 for the District Manager.
District Manager McKnight stated that she will prepare her goals for the next fiscal year. Director Carr does
not want to have a committee review the District Manager. The Board directs the District Manager to
prepare goals that the Board can review next month.
Motion made by Director Moss, seconded by Secretary/Treasurer Carr, to approve a salary increase of
$5,000 per year effective on September 19, 2012 and include the District Manager in the 1.5% COLA on her
current salary.
Director Armstrong would like to grant the merit on October 1, 2012 based on her salary after her merit
increase.
The Mover and Second agreed to change the main motion to approve a salary increase for the District
Manager of $5,000 effective on September 19, 2012 and a 1.5% COLA increase on October 1, 2012 to be
calculated on the new salary.

INTERLOCAL COOPERATION AGREEMENT FOR
SHARED GOVERNANCE COMMUNICATIONS & DISPATCH SERVICES SYSTEM
This Interlocal Cooperation Agreement for Shared Governance Communications and
Dispatch Services System, hereinafter referred to as "Agreement", is made by and between Denton
County, a political subdivision of the State of Texas, hereinafter referred to as the “County”, and
Name of Agency:
hereinafter referred to as “Agency”.
WHEREAS, the County is a duly organized political subdivision of the State of Texas
engaged in the administration of county government and related services for the benefit of the
citizens of Denton County, Texas; and
WHEREAS, the Agency is duly organized and operating under the laws of the State of
Texas engaged in the provision of municipal government and/or related services for the
benefit of the citizens of Agency; and
WHEREAS, parties agree that the utilization of combined communications and dispatch
services system will be in the best interests of both the County and the Agency,
WHEREAS, the County and the Agency mutually desire to be subject to the provisions of
the Interlocal Cooperation Act of the V.T.C.A. Government Code, Chapter 791; and
NOW THEREFORE, the County and the Agency, for the mutual consideration
hereinafter stated, agree and understand as follows:
1. PURPOSE. The Denton County Sheriff (“Sheriff”) has the facilities to provide
emergency telecommunications and dispatch services throughout Denton County. The Agency
wishes to utilize the Sheriff’s available telecommunications and dispatch services (“Services”)
during the term of this agreement.
2. ADVISORY BOARD. The Denton County Sheriff’s Office will establish an
Advisory Board for the Shared Governance Communication and Dispatch System “Advisory
Board”. The membership of the board shall be the Chief of each Agency, or designee. The
Advisory Board may advise and make recommendations to the Sheriff and the Sheriff’s Office
on matters relating to the Communications Center, as well as the recommendations for the
Annual Agency Workload and Cost Statistics, within the limitations set forth in paragraph 6.1,
herein.
3. TERM OF AGREEMENT. The initial term of this Agreement shall be for a one year
period beginning October 1, 2012 and ending on September 30, 2013.
4. TERMINATION OF AGREEMENT. Either party may terminate this agreement,
with or without cause, after providing ninety (90) days written notice to the other party.

5. ANNUAL SERVICE FEE. Each Agency shall pay to the County a fee for service for
services based on the workload generated by the Agency.
5.1. Agency shall pay to County the Total Amount more fully described on
Exhibit “A”, the Agency Workload and Cost Statistics.
5.2. The Agency shall complete Exhibit “B”, Agency Payment Worksheet, to
identify the payment terms preferred by Agency. Agency is responsible
for sending payments to County.
5.3. The fee for service will be based on the pro rata share of the workload
generated by the Agency.
5.4. County agrees to provide Agency a proposed service fees for the next
budget/fiscal year as agreed by the parties.
5.5 If this Agreement is terminated prior to the expiration of the term of the
Agreement, payment shall be pro-rated by written agreement between
the parties.
6. COUNTY SERVICES AND RESPONSIBILITIES. The County agrees to provide the
following services and responsibilities:
6.1 The Sheriff shall have the sole discretion as to the method of providing
the Services including, but not limited to the order of response to calls, and shall be the sole
judge as to the most expeditious and effective manner of handling and responding to calls for
service or the rendering thereof. The Sheriff shall have the sole discretion as to the method and
final decision regarding the annual workload and cost statistics. The Sheriff will devote
sufficient time to insure the performance of all duties and obligations set forth herein.
6.2 County shall furnish full-time communications services including a
twenty-four (24) hours a day, seven (7) days a week public safety answering point, radio
services, dispatching services, or law enforcement transmission originating from AGENCY
requesting law enforcement and fire protection services and access to local, regional, state, and
national data bases and telecommunications systems.
6.3 The services provided by County include the following:
6.3.1 twenty-four (24) hours a day, seven (7) days a week public safety
answering point;
6.3.2. receiving emergency and routine calls for law enforcement, fire,
and medical services;
6.3.3 directing a response to said calls by dispatching the appropriate
law enforcement, fire, and medical services;
6.3.4. providing on-going communication support to the emergency
personnel in the field; and
6.3.5 updating, maintaining, and managing the County owned radio
communications system, computer systems, support files, and
resource materials necessary to accomplish the above.
6.4 County may add new Agencies not currently served by Denton County at
the discretion of Denton County and the Denton County Sheriff’s Office.
7. AGENCY RESPONSIBILITIES. The Agency agrees to the following responsibilities:
7.1 Providing accurate current GIS data of the corporate limits and
extraterritorial jurisdiction of the Agency.
7.2 Furnish County with a current list of all Officers and Reserves authorized

by Agency to use the communications system.
7.3 Agency is responsible for the costs and upgrades associated with
maintaining Agency’s communication equipment.
7.4 Agency agrees to abide by all laws of the United States and the State of
Texas and all present or hereafter approved rules, policies and
procedures of TLETS, NLETS, TCIC, NCIC and any other system now or in
the future associated with TLETS concerning the collection, storage,
processing, retrieval, dissemination and exchange of information for
criminal justice purposes
7.5 Adherence to all Sheriff’s Office communications rules and regulations.
7.6 Agency agrees to provide all necessary and required TLETS paperwork. See
Exhibit “C”.
7.7 Appoint representative and agree to participate in the Advisory Board.
7.8 Agency is responsible for sending payments to County as more fully
described in Exhibit “B” to this Agreement.
8. AGREEMENT. The parties acknowledge they have read and understand and
intend to be bound by the terms and conditions of this Agreement. This Agreement contains
the entire understanding between the parties concerning the subject matter hereof. No prior
understandings, whether verbal or written, between the parties or their agents are enforceable
unless included in writing in this agreement. This Agreement may be amended only by written
instrument signed by both parties.
9. AGREEMENT LIASONS. Each party to this agreement shall designate a Liaison to
insure the performance of all duties and obligations of the parties. The Liaison for each party
shall devote sufficient time and attention to the execution of said duties on behalf of the Party
to ensure full compliance with the terms and conditions of this Agreement.
10. ASSIGNMENT. Neither party shall assign, transfer, or sub-contract any of its
rights, burdens, duties, or obligations under this Agreement without the prior written permission
of the other party to this Agreement.
11. AGENCY LIABILITY. The Agency understands and agrees that the Agency, its
employees, servants, agents, and representatives shall at no time represent themselves to be
employees, servants, agents, and/or representatives of the County. The Agency shall not be
required to indemnify nor defend County for any liability arising out of the wrongful acts of
employees or agents of County to the extent allowed by Texas law.
12. COUNTY LIABILITY. The County understands and agrees that the County, its
employees, servants, agents, and representatives shall at no time represent themselves to be
employees, servants, agents, and/or representatives of the Agency. The County shall not be
required to indemnify nor defend Agency for any liability arising out of the wrongful acts of
employees or agents of Agency to the extent allowed by Texas law.
13. DISPUTES/RECOURSE. County and Agency agree that any disputes or
disagreements that may arise which are not resolved at the staff level by the parties should be
referred to the Appointed Liaisons for each entity. Any further disputes arising from the failure
of either Agency or County to perform and/or agree on proportionate reduction in fees shall
be submitted to mediation, with the parties splitting the mediation fees equally. It is further
agreed and understood that the scope of matters to be submitted to dispute mediation as
referenced above is limited to disputes concerning sufficiency of performance and duty to pay

or entitlement, if any, to any reduced fee or compensation. Any other disputes or conflicts
involving damages or claimed remedies outside the scope of sufficiency of performance and
compensation adjustment shall be referred to a court of competent jurisdiction in Denton
County, Texas.

following Exhibits, which are hereby incorporated by reference.

15. MULTIPLE ORIGINALS. It is understood and agreed that this Agreement may be
executed in a number of identical counterparts, each of which shall be deemed an original for
all purposes.
16. NOTICES. All notices, demands or other writings may be delivered by either party

17. SEVERABILITY. The validity of this Agreement and/or any of its terms or
provisions, as well as the rights and duties of the parties hereto, shall be governed by the laws
of the State of Texas. Further, this Agreement shall be preformed and all compensation payable
in Denton County, Texas. In the event that any portion of this Agreement shall be found to be
contrary to law, it is the intent of the parties hereto that the remaining portions shall remain
valid and in full force and effect to the extent possible.
18. THIRD PARTY. This Agreement is made for the express purpose of providing
communications and dispatch services, which both parties recognize to be a governmental
function. Except as provided in this Agreement, neither party assumes any liability beyond
that provided by law. This Agreement is not intended to create any liability for the benefit of
third parties.
19. VENUE. This agreement will be governed and construed according to the laws
of the State of Texas. This agreement shall be performed in Denton County, Texas.
20. WAIVER. The failure of County or Agency to insist upon the performance of
any term or provision of this Agreement or to exercise or enforce any right herein conferred,
or the waiver of a breach of any provision of this Agreement by either party, shall not be
construed as a waiver or relinquishment to any extent of either party’s right to assert or rely
upon any such term or right, or future breach of such provision, on any future occasion.
21. AUTHORIZED OFFICIALS. Each party has the full power and authority to enter
into and perform this Agreement. The persons executing this Agreement represent they have
been properly authorized to sign on behalf of their governmental entity.
22. CURRENT FUNDS. All payments made by Agency to County pursuant to this
Agreement shall be from current revenues available to Agency.
23. DISPATCH & COMMUNICATION RECORDS. The parties acknowledge that the
Denton County Sheriff’s Office may release dispatch and communication records of Agency
pursuant to the Texas Public Information Act until such a time that the parties agree to transfer
such responsibility to Agency.

NON - TWENTY-FOUR HOUR TERMINAL AGENCY AGREEMENT 2012-13

Non Twenty-Four Hour Terminal Agency
This document constitutes an agreement between the following parties:
The Twenty-Four Hour Terminal Agency agrees to make entries into the Texas Crime Information Center
(TCIC) and the National Crime Information Center (NCIC) computers for the Non Twenty-Four Hour
Terminal Agency.
All records must be entered with the Twenty-Four Hour Agency’s ORI, and all case reports and original
warrants must be held at the Twenty-Four Hour Agency for hit confirmation purposes.
The Non Twenty-Four Hour Agency agrees to abide by all laws of the United States and the State of
Texas and all present or hereafter approved rules, policies and procedures of TLETS, NLETS, TCIC,
NCIC and any other system now or in the future associated with TLETS concerning the collection,
storage, processing, retrieval, dissemination and exchange of information for criminal justice purposes.
The Twenty-Four Hour Agency reserves the right to suspend service to the Non Twenty-Four Hour
Agency which may include canceling of records entered for the Non Twenty-Four Hour Agency when
applicable policies are violated. The Twenty-Four Hour Agency may reinstate service following such
instances upon receipt of satisfactory assurances that such violations have been corrected.
In order to comply with NCIC policies established by the NCIC Advisory Policy Board, the Non Twenty-
Four Hour Agency agrees to maintain accurate records of all TCIC/NCIC entries made through the
Twenty-Four Hour Agency and to immediately notify the Twenty-Four Hour Agency of any changes in the
status of those reports to include the need for cancellation, addition, deletion or modification of
information. The Twenty-Four Hour Agency agrees to enter, update and remove all records for the Non
Twenty-Four Hour Agency on a timely basis, as defined by NCIC.
In order to comply with NCIC Validation requirements, the Non Twenty-Four Hour Agency agrees to
perform all validation procedures as required by NCIC on all records entered through the Twenty-Four
Hour Agency.
Either the Twenty-Four Hour Agency or the Non Twenty-Four Hour Agency may, upon thirty days written
notice, discontinue this agreement.
To the extent allowed by the laws of the State of Texas, the Non Twenty-Four Hour Agency agrees to
indemnify and save harmless the Twenty-Four Hour Agency as well as the DPS, its Director and
employees from and against all claims, demands, actions and suits, including but not limited to any
liability for damages by reason of or arising out of any false arrests or imprisonment or any cause of the
Non Twenty-Four Hour Agency or its employees in the exercise of the enjoyment of this Agreement.
In witness whereof, the parties hereto caused this agreement to be executed by the proper officers and officials.

Upon collection of one or more Utility Fees by Town,such Utility Fees shall,within ten (10) working days, be deposited by Town into an account designated
by MUDs (hereinafter "MUDs Account").Town shall continue to collect and to deposit such Utility Fees into MUDs Account in accordance with the existing
agreement between the MUDs until such time as the total amount paid into that account equals $3,260,000.Town shall collect and deposit a minimum of one
hundred fifty (150)Utility Fees each year, for a total of five (5)successive years commencing on May 7, 2008 (hereinafter "Collection Period").In the event
that more than 150 Utility Fees are collected by Town in anyone or more years of the Collection Period,such Utility Fees in excess of the 150 Utility Fees
required under this Section shall be carried over to the following year or years and shall be a credit toward the total Utility Fees due for such subsequent year
or years.
In the event that fewer than 150 Utility Fees are collected by Town during anyone or more year(s) of the Collection Period, including those Utility Fees carried
over from any given year in which more than 150 Utility Fees were collected by Town,Town shall within ten (10) working days of collection of such Utility
Fees from Developer, pay into MUDs Account the difference between the amount that would have been paid had 150 Access Fees been paid at $2,300 each
and the amount of Utility Fees actually collected by Town.On or about May 7, 2013,Town shall calculate the total amount of Utility Fees paid to MUDs.Town
shall provide MUDs written notice of any amount paid less than $3,260,000 (the "Utility Fee Remainder").

MINIMUM QUALIFICATIONS
The following are minimum qualifications for appointments to both regular full-time or part-time
Fire Department positions for the Town of Trophy Club. All applicants must answer yes or no and
submit along with the Notice of Interest in order to be considered for an open position.
1. Do you possess a current Basic Firefighter Certification through the Texas
Commission on Fire Protection?
2. Do you posses a current certification as a paramedic in Texas?
3. Have you reached your 21st birthday prior to appointment to the position of
firefighter?
4. Are you a citizen of the United States?
5. Are you a high school graduate, or possess a GED?
6. Have you been convicted of a felony offense?
7. Have you received a DWI conviction or arrest for DWI in the past 10 years?
8. Are you on community supervision or probation or under indictment for any offense?
9. Have you been convicted of any offense of the grade of Class B misdemeanor or
above?
10. Do you have a valid Texas driver’s license and have not been convicted of more than
three (3) moving violations and/or accidents within a 24-month period?

SECTION 7 Part-time Fire Fighter Special Requirements
Prior to being assigned to a shift and working all part-time firefighter must adhere to the following:
1. Must be credentialed to work under the current Trophy Club Medical Control Group or willing to obtain
accreditation at the applicant’s expense.
2. Must have reviewed the part-time applicant packet; completed and returned Notice of Interest
Application;
3. Must have an acceptable driving record;
4. Must pass a drug test;
5. Must have off duty work authorization form or letter from current employer*:
a. Must state current status
b. Must be clear from any disciplinary actions
c. Must be signed by Chief Officer
6. Must successfully complete an oral interview with the Fire Chief;
7. Candidate must complete orientation with Human Resources;
8. Candidate must have completed an eight (8) hour orientation session with the Training Coordinator:
a. All training records will be evaluated and filed
b. Demonstrate knowledge of Town lay-out
c. Must demonstrate knowledge of equipment
9. After successful completion of orientation the firefighter will be assigned to an FTO (Field Training
Officer) shift for a 24 hour shift;
10. After successful completion of an FTO shift, and with a recommendation from the training coordinator
and FTO, the firefighter will be assigned to the active duty pool for further assignment.

These instructions are provided as a guide to assist you in properly completing your PERSONAL HISTORY
STATEMENT.
During your pre-employment process with the Trophy Club Fire Department, it is important to dress
appropriately (as you would for any job interview). Unless otherwise instructed, T-Shirts, shorts, tennis shoes,
sweat clothes, etc., are not to be worn to any of your appointments for interviews or oral review boards. You
will need to let us know promptly if you cannot make an appointment due to an emergency.
IT IS ESSENTIAL THAT THE INFORMATION BE CORRECT AND COMPLETE
FALSIFICATION OR OMISSION OF ANY KIND MAY BE GROUNDS FOR REMOVAL FROM THE
SELECTION PROCESS.
1. Your Personal History Statement should be HAND PRINTED legibly in black ink.
2. Answer all questions completely. If a question does not apply to you, enter “N/A” in the space
provided.
3. Avoid errors by reading the directions carefully before making any entries on the form. Be
sure your information is correct and in sequence before you begin.
4. YOU ARE RESPONSIBLE FOR OBTAINING CORRECT ADDRESSES, INCLUDING ZIP
CODES. If you are not sure of an address, check it for personal verification. Your local library
or Internet service provider may have a directory service or copies of the required directories.
Include the area code with the telephone numbers.
5. If there is insufficient space on the Personal History Statement form, attach extra sheets.
Be sure to reference the relevant section and question before continuing your answer.
6. If documents must be mailed, notify the background investigator.
If you fail to return the Personal History Statement within ten (10) business days from the date of
issuance, you will be removed from the selection process.
Do NOT call the Fire Department or your background investigator to check on the progress or status
of your application. Background Officers will notify you of any developments.
Riding with Fire Department employees is not permitted while you are in the application process. Should you
wish to withdraw your application at any time, please notify us in writing so that we may devote our efforts
toward those applicants still desiring employment with the Trophy Club Fire Department.
